The Efficiency of Secondary Blood Test for the Judgment of Donor Suitability of Allogenic Bone Graft

Abstract
Purpose: Considering the incubation period of the viral disease, the second blood test could be performed at least 3~6 months after the first test to ensure the suitability of the allogenic bone graft from the living donor in a few tissue banks. The allograft would be discarded unless the second test was available and confirmed, the purpose of this study was to analyze the efficiency of secondary blood test for the judgment of donor suitability of allogenic bone graft.

Materials and Methods: During the period from April 2005 to May 2011, allogenic bones were retrieved from 321 donors including 99 donors with hip replacement arthroplasty and 222 donors with knee replacement arthroplasty. The average age of donors is 69 years (range, 40~88 years) and they were 42 males and 279 females. The first blood test including hepatitis B, hepatitis C, AIDS and Syphilis were checked before retrieving grafts. The results were analyzed after the second test performed at least 3~6 months after the first because of the incubation period of the viral disease.

Results: There was none with positive result of the second test among 321 donors who have the negative results in the first test. However, the grafts from 28 donors (8.7%) were discarded, because the second test was not checked or refused.

Conslusion: It was extremely rare that the possibility of the second test shows the positive results for donors who have the first test with negative results. However, not a few grafts were discarded only because the second test was not available. In terms of economics and ethics, the second test may not be necessary or if necessary, it should be considered to perform the single test such as nucleic acid amplification testing (NAT) as a test for infectious disease for judgment of donor suitability of allogenic bone.
